The consultation appointment is important for Dr. Truitt to perform an oral examination, evaluate your X-rays, review your dental and medical history, make a proper diagnosis, and provide a treatment plan that is appropriate for your individual needs.
IMPORTANT: All patients under the age of 18 years old must be accompanied by a parent or legal guardian.

Please print off and fill out all of the forms below and bring to your first appointment, along with alist of your medications, Name and Phone Number of any medical specialists you see and the referral slip (if given to you) from your dentist. Also please bring a photo ID and your insurance cards.
