Payment Options

INSURANCE

We will contact your insurance company before your treatment to estimate your out of pocket expenses as well as file all in-network insurances for you. You will be given a treatment plan at the initial consultation visit. All co-pays and deductibles are due on the date of treatment.

Your insurance company determines whether your procedure is covered under your medical or dental insurance. Please be prepared to have both your medical and dental insurance information with you when you arrive for your first visit, including your medical and dental insurance cards.

We participate with the following medical insurance plans:

Cigna Open Access Plus

We do not file to any medical insurances with which we are out of network.

We are also happy to see patients with no dental coverage.

Fees for services provided are due at time of care.

We do not participate with Medicare or any Medicare supplements/products.

CARE CREDIT

We do accept Care Credit payments. You can apply for Care Credit at www.carecredit.com

When you pay for services provided at our office within six months of date of service, there is no interest charged (for payments over $200). Care Credit can be used at many medical facilities including dental offices, cosmetic surgeons, veterinary offices, for hearing aids, Lasik and vision care, and many more.

PAYMENT TYPES

We accept most major credit cards, Care Credit, checks and cash. There is a returned check fee of $35.00.

All co-pays and deductibles are due at time of treatment.
